Performing CAPA Investigations

Purpose

As an Investigator, you can evaluate a Safety Observation to confirm the issue, identify contributing factors, and determine required actions. This supports consistent review and ensures each observation receives appropriate follow‑up.

Prerequisites

• Installation Package: CQ Core and CQ EHS Extension

• Product: SafetyQuest

• Function: Observations

• User Profile: Standard User

• Permissions: CQ Base User PSG and CQ EHS Base User PSG and CQ Safety Observation Basic User PSG or CQ Safety Observation User PSG or CQ Safety Observation Manager PSG

• Configurations: Root Causes

• Record Stage: Under Investigation

Steps

1. Open the Email notification or a Home tab notification.

2. Click Perform Investigation in the Next Best Actions section.

3. Specify 5W2H Problem Solving summaries. 5W2H can be generated via CQ.AI.

4. Add Investigation Tools and Root Cause(s).

5. Click Save & Next.

6. Add Action Plans.

7. Specify Investigation Summary and Conclusion.

8. Click Save.

Expected Outcome

• The Investigation results appear on the Investigation tab.

• The Submit Investigation button appears in the Next Best Actions section if the Investigation Summary is specified.

• Updates can be made at anytime to the Investigation prior to submission.
